Our CAREGivers will work with your hospital discharge planning staff to develop a detailed plan of care that will be used by family and other home caregivers to ensure your loved one has a successful recovery.
A Home Instead CAREGiver will personally assist you from your hospital bed to the comfort of your own home and help you settle in.
Simple tasks such as changing your bed linen or doing the laundry can be physically impossible when you are first discharged from hospital. A Home Instead CAREGiver will ensure your bed is made and you have clean sheets, towels and a tidy house while you focus on your recovery.
Our CAREGivers will help you organise and keep track of your medication to ensure that they are taken as directed. They will also help you avoid any adverse effects or monitor any reactions you may experience.
Often, after a visit to hospital, our tastebuds and appetite can also take a while to recover. Our CAREGivers can assist you with meal planning, shopping, preparation and cooking to ensure you are eating plenty of wholesome, nutritious foods and staying hydrated by drinking plenty of water.
Warning sign monitoring
Our CAREGivers are familiar with and trained to watch for warning signs and take appropriate action to help prevent complications, setbacks or hospital readmissions.
Our CAREGivers are trained to record specific information on your post-discharge care and recovery. Keeping a record of your transition care and recovery progress is important to share with your doctor and can be used to update, if necessary, your original discharge plan of care.
Our CAREGivers will help you keep track of all of your follow-up medical appointments. A CAREGiver can personally drive and escort you to your appointments or arrange transport for you.
