As a Care Manager you will be responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients in and around Albury, Wodonga and surrounding areas, embodying our mission: “To us, it’s personal.”

Key Responsibilities:

  • Create, manage, and review Home Care Package (HCP) budgets, liaising with approved service providers and allied health professionals.
  • Plan, coordinate, and review client goals and care plans, meeting with clients and families to formalize agreements on care services and associated costs.
  • Accurately document all client interactions, share relevant information with team members, and provide feedback on CAREGiver performance to ensure effective communication.
  • Ensure compliance with industry regulations and effectively use My Aged Care and other relevant HCP apps and portals.
  • Work collaboratively with the team to support day-to-day operations that foster a positive and supportive work environment.

About you:

  • Existing experience in care management is essential
  • Minimum Cert IV in Age Care / Individual Support
  • Able to travel in and around Albury, Wodonga and surrounding area
  • Highly efficient and organized with proven time management skills
  • Able to work under pressure and multitask shifting priorities
  • Strong written and verbal communication skills
  • Must hold a valid Australian Driver’s License
